Physicals must be dated after May 1st, 2011. Please fill out the top portion of the physical form before your appointment. I have attached a copy of the physical form and the complete instructions from the schools website.
Completed physicals are due to your coach by Monday, August 8th, 2011, the first day of fall football. THIS IS A VERY IMPORTANT DATE! Any player, without a submitted physical on file with the Elk Grove Athletic Department, will not be allowed to participate in the season's first practice.
